INSURANCE:
Center for Advanced Orthopaedics & Sports Medicine, PC, participates with Medicare and a variety of commercial and managed care insurance plans. Worker’s Compensation information needs to be verified prior to the appointment. Please contact our office directly to see if we participate with your specific insurance plan.

Please make sure you have a current, valid insurance card at the time of your visit. If you do not present an insurance card at the time of your visit, you will be required to pay for your appointment, in full, at that time or you may reschedule your appointment.

Co-payments are required at the time of your visit. For your convenience we accept cash, checks, Mastercard and Visa.

APPOINTMENTS:
New patients should arrive at least 15 minutes before their scheduled appointment to complete the necessary paperwork.

You may also download the patient registration and medical history forms from this site and bring the completed forms with you to your initial visit. Please see the “Patient Forms” section of this website for more information.

In the event you need to cancel an appointment, we would appreciate at least 24 hr notice so that we may adjust our schedules accordingly.

PRESCRIPTION REFILLS:
Prescription refills will only be called in during regular office hours, Monday – Friday 8am- 5pm. We feel that it is in the best interest of patient care to have a patient’s medical record available to the covering physician when these prescriptions are renewed.
